Bug Reporting tool

I would like this in V1 but if we can't do it then lets push it out. Basically i want a small pullout tab on the page to report bugs, this pullout is a basic form that will submit to GitHub as an issue.

If you can get the form submission to Github working then I can sort out the UX/UI.

If we do this we should create a new label for website submitted bugs.

Otherwise our other option is I create another backend api we can utilize just to submit issues to github. - This one is a bit more work and would have to be post v1
