Create XML Incident Definition

I will translate the HTML versions of the assessment forms into XML, which will permit embedding much more incident meta data.

This XML file will be the basis for XML schemas that will drive the assessment form, phases, the sign-up form, etc.
I anticipate that the schema will need regular updating and new versioning without requiring us to update previous incidents.
