This PR introduces the --group-only / --no-group and --skip-existing processing options in response to issues #56, #57, and #58.

The goal of these additional options is to allow for more control when processing larger sets of cases; in particular, this will enable -- and separate -- incremental processing for individual cases, and subsequent creation of group-based statistics and reports.
