Add template for bug report

To enhance Django Girls' issue tracking and streamline the bug replication process, this PR suggests adding GitHub's beta issue forms. This upgrade should make the process more efficient by automatically adding labels and helping structure the bug reporting process to give the rest of the team and interested contributors important info to tackle bugs more quickly.

Actions needed from maintainers

• Create a triage label

Not really needed to merge this. I just thought it could be helpful to differentiate between issues that the DG team has triaged and provided additional details on, versus those that haven't been.
