This PR introduces the relevant tables and SQL to create analytics for SA CHT.

New Table

  • Response table (Daily responses to VMMC forms)
  • Message table (messages sent and received)

Modifications

Patient Table

  • vmmc_no
  • enrollment_facility
  • enrollment_location
  • age

Data Record Table

  • patient_sms_response
