Update Readme with app registration instruction via Azure Portal

The readme sends us to App Registration Portal, which is out-of-service.
The new way is to register the app via Azure Portal. There are a few settings that have to be set, to make this sample work.
This should be documented, so new useres of the sample can quickly start using it
