Use a single field to represent a user's personal name

With reference to this article, and following discussion on Slack, we will represent a user's personal name using a single field in both the "user" table and in front-end applications, foregoing the more traditional first name / last name split.

This work will be expedited to (partially) resolve #15 (signup form only - profile form to be done separately).
