Student-Facing Survey Embed UX needs alignment with Legacy functionality #70

The UX for accessing surveys from a student perspective needs a few finishing touches.

Migrated Survey Embeds when viewed from Student Perspective should...

be wrapped in a Survey label (previously "My Response")
Have the Survey Title as the clickable link
'Submit Survey' should be the button label at the bottom of the survey. Clicking Submit should 'lock in' the results and display the students responses in read-only state.

Depending on #69 :

Survey must open in New Tab (Attempts to do so in delivery but fails, Preview keeps in same tab)
