Dataverse - Duplicate Record Origin

I have issue finding an origin of the record named "Z. H. - booking resource".
How is it possible there is another record named "Z. H. - booking resource"? Why is the Member value different from Name?

These team member records were created by flow (Dataverse connector) - both records were created by admin_networg at the same time and it runs in context of this user, that would mean that the records were created by flow... However, there exists only one flow run at this time:

You can see from the screenshots below that the record created in this run is the one named "D. T. - booking resource":

(The other created record has different name):
