This is the magento module creator 0.0.9.1 that you can find here http://www.magentocommerce.com/wiki/5_-_modules_and_development/0_-_module_development_in_magento/custom_module_with_custom_database_table, but the link is broken. The version 0.0.9.2 works, but it separates the module in two part, the backend and the frontend. The backend part does not work because there should be some path error in the configurations file. The 0.0.9.1 still works out of the box.
