Bug: SUM should not affect instruction accesses. #2

The SUM bit (permit Supervisor User Memory access) does not apply to instruction accesses, so...

The fetch stage should not allow the supervisor to access user mode cache lines.
The execute stage should not allow supervisor to fetch user executable memory into the instruction cache.
