50+ users affected.

1-click-child-links is not working intermittently for 50+ users.
It is currently not pulling up the templates 80% of the time and hence not creating any child tasks.

**Now, it has stopped working entirely. Please help. **

Templates are already in place.

How can this be resolved. Does it need any additional configuration (note: several scrum teams have been using this so far), esp. add users in TFS?
Please advise.

It seems that you don't have anything in the description fields to instruct 1-click to which work item to add the new items. Look here for examples: https://marketplace.visualstudio.com/items?itemName=ruifig.vsts-work-item-one-click-child-links.
