Fill in missing CHANGELOG releases (#44940)

Fixes #32790

Use a tool to merge version-specific H2 sections from v14, v15, and v16
of the CHANGELOG. Also changes version-specific H3 sections to H2s, and
always includes a major, minor, and patch version, in order to keep the
CHANGELOG consistent.
