Bug 1
This happened in the latest version of the game.
In the Forest, I got attacked by a bat and I clipped through the ceiling. There was no way of getting out.

This will be related in part to #2456.

Thanks for reporting this. There are several unfortunate edge cases with our collision system that put the player in broken states such that you found unfortunately.
