About generate image when insert nurse and patient datas

I have some problem about create image url when save datas.
Would you explain about this?

well, first, you have to create image and save it on frontend directory.
then name it depends on its data(name,type etc)
and when show the datas, from data, output images

On here, frontend returns state value,and on backend analyze it, and generate file
