firebird_fdw release 0.4.0

firebird_fdw 0.4.0 has been released with following changes:

• add function firebird_fdw_version()
• add TAP tests
• fix bug in INSERT ... RETURNING ...
• miscellaneous minor bug fixes and code cleanup

This release requires libfq version 0.3.0.
