All of these changes look fine and correct to me, but I think the process itself needs to be streamlined more. This has steps for both using the ticket form and for sending an email. Could we move to all email, or all tickets? I think it's confusing to split the process.

Also, my recommendation is to update the form so the questions are just field names instead of questions:

e.g., What is your organization's website? becomes Organization website

I think it would make the form easier to read.
