Mobile: Add ability to add multiple children for birthday check

This is a continuation of the work started in issue #885. A parent might have children who are ready for preschool and others who are ready for infant/toddler programs. While we can't promise that children will be placed at the same site, parents still probably want to check for options that are appropriate for their children.
