Capture/show "How did you find out about the CUTGroup?" information on tester profile page

When we have a new tester sign-up, we have an optional question "How did you find out about the CUTGroup?" We would like to make sure we are capturing these responses in Patterns and showing them on the tester profile page.

In addition, and related to issue #183, I would like to be able to see these responses when analyzing recent sign-up information.
