Adds support for the Tandem t:slim insulin pump

The following is not yet supported by the Tandem driver:

  • "dotted line"/suppressed info for temp basals and suspends
  • pump settings & settings history
  • device events: suspend/resume, reservoir changes, prime events, alarms
  • CGM & CGM-related data from the G4
  • Uploading during a temp basal

Other changes:

  • adds new annotation final-basal/fabricated-from-schedule for Insulet & Tandem final basals, such that they will be modified to reflect reality on next upload as long as jellyfish v0.11.1+ is in use.
  • cleans up UI descriptions of devices
