The usability test findings Github issue template needs to be updated to better reflect our current practice and also to include metadata guidance.

The usability testing findings Github issue template has been updated with our latest process and added "Labels and metadata" guidance. I'll share it with the development team to make sure all looks good.

I presented the template to the team in CS UX sync today and general consensus was that it looks good. This issue can be marked done.
